Can a hiatal hernia cause refered right shoulder pain?

Not normally. A hiatal hernia can caused referred pain to the left shoulder rarely but not normally to the right shoulder. One way it could cause some referred pain would be if the hiatal hernia is causing so much irritation to your diaphragm that its spasming on the right, which could lend itself to some referred pain, but this is not very common. I hope this helps!
