In brief: Where? What r u eat
Depends on where you are, what you eat, and drink.
Safest plan in a place where real risk for food poisoning, traveller's diarrhea, hepatitis, etc. Is "if you can't peel it, or boil it, forget it". Often issue is h2o-use bottled water to brush teeth; no ice in drinks. Salad, ceviche, raw anything is suspect, even in 5 star hotel. Remember u r in a 5 star hotel, but staff don't live in it.
