In brief: Plastic Surgery?

You may need plastic surgery to make your face look thinner. Orthodontics will change the alignment of the teeth but can't change your skeletal or muscular structures.
You will probably need to consult an orthognathic surgeon or a plastic surgeon in conjunction with your orthodontist.

In brief: Narrowing faces

Facial 'width' is determined at several levels-cheekbones, cheeks, jaw.
And if your facial structure is 'ovoid' braces (ie tooth level treatment) cannot change it to tapering or square tapering. Discuss these specifics at length with an oral surgeon. The soft tissue envelope is a reflection of overall body weight. Braces cannot 'narrow' the facial bony skeleton--nor can they 'thin' a 'plump' face.

Generally this is not accomplished via orthodontics. You may have a distorted view of yourself. Discuss with a PCP or mental health professional, best of luck!

In brief: Orthognathic

If you measured your face from zygoma to zygoma, the width can be reduced with bone reduction plastic surgery.
If you measured the width from mandible angle to angle, then thinning the madible muscle might make your face thinner. If you have a short face and need face to look longer, then orthognathic orthodonosia with possible surgery will help.
