In brief: Limited treatments
This depends on the type of leukemia and prior treatments but often when leukemia recurs after a bone marrow transplant the treatment options are limited.
In many situations the treatment is focused on controlling the leukemia and symptoms since cure is rare at that point. This is something that should be discussed with the transplant physician.

In brief: Difficult case
Need to get salvage therapy.
Option would be salvage with a second allo transplant ( myeloablative or reduced intensity) or donor lymphocyte infusion can be considered. Salvage chemo and clinical trials also will be considered.
