When does a person get esrd? Does end-stage mean he'll never recover from it?

In brief: When kidneys fail...
Esrd is a term that is indicative of kidney failure that is severe enough to require either dialysis or transplant to prevent death.
It has its roots in the medicare benefit available to all since the 1970s. It is a bad clinical term since you as a person are not end stage but your kidneys are. By definition it is not recoverable.
