Cause is important.
Site must be examined carefully for foreign materials, cleaned very thoroughly, any dead tissue removed from the full depth of the wound. Best to see physician due to risks of infection and problems. Immediately do your best to clean, rinse, perhaps use peroxide (dilute to 1/2 strgth), cover with antibiotic ointment and dressing and see your doctor.

If the puncture, however, was created by a nail, wood splinter, cat/dog bite, etc there is a higher risk of infection and i would recommend seeing a doctor to be placed on an oral antibiotic. You should also know your tetanus immunization status and receive a booster if that is not up to date.
