In brief: Throat lump

It can be associated with acid reflux and i see that you have heartburn and you smoke, which does not help it.
The lump may also be from post nasal drip if you have nasal allergies. I will bet on acid reflux, you need to change your diet and stop smoking and maybe take some otc antacids.
