In some cases the patient can be slightly seated up if cannot lie flat; microscope has best view of cataract when patient lying flat.

It is possible to sit up for the most part, but one must tip their head back and look straight up to get the microscope in position to perform the surgery.
So you could sit up but tip your head back perpendicular to the floor. Body positioning is not the issue, it is the eye looking up into the microscope and avoiding the bones of the orbit to get access to the eyeball to perform surgery.

I can't imagine doing it with a patient sitting up, though.
