It depends how small - there is individual variation just as there is with earlobe or breast size.
Very small testes may be due to many different factors, some of which lead to low testosterone as ex adrenal steroid weight lifter abusers, or to no sperm as men with klinefelters syndrome.

It is more likely that there is a problem if they had been larger and then shrunk to smaller size

The reverse may be true. Low testosterone may lead to small testicles. Use of androgenic steroids can do this. How do you know your testicles are small? Have you measured them? (ultrasound is very accurate in this regard and does't hurt)
