In brief: Q waves

Some heart attacks do not show and even ones that do, may only be temporary.
That said, q waves, if of sufficient voltage and if they follow known patterns, are the way of diagnosing heart attack by ekg. (caution: not all q waves are indicative of heart attack!).

Agree. Others may show by loss of voltage, axis shift, or a new conduction delay (bundle branch block)

In brief: Q waves

On ekg but not all previous heart attacks will show up.
Echocardiogram is often more sensitive.
