Diuretics like Lasix (furosemide) can be used but support of the cardiac output is essential. Acute care is usually in the ICU with IV meds. Chronic heart failure managed as an outpatient with diuretics, ace inhibitors, and other cardiac meds requires close follow-up of blood pressure, electrolytes, and kidney function.

In heart failure, there is often extra fluid in the body that isn't being pumped by the heart to the kidneys (to be excreted) because it is often in the tissues,not in the blood vessels, and the heart doesn't pump well. But using a diuretic may also result in less fluid in the blood vessels ( with even lower blood pressure). So diuretics must be carefully monitored. They are very helpful when someone is very short of breath because of extra fluid in the lungs, along with other medications.
