Inflamed diverticuli may affect adjacent tissues with the inflammation of these producing a wide spectrum of symptoms.
They may perforate and produce localized abscesses, peritonitis, sepsis, formation of fistulous tracts to other organs, scarring and adhesions may create intestinal obstruction. If appropriately managed, however, these are uncommon.

80-85% with colonic divertticulosis never present with clinical problems.
Of other 15-20%, roughly 3/4 develop diverticulitis & 1/4 have some degree of bleeding. Early on, diverticuli that inflame (diverticulitis) may cause local tenderness, fever, nausea/vomiting, ileus. If unresolved, diverticulitis can lead to abscess (pus-pocket), free perforation with peritonitis, fistulization, obstruction.
