When a mass or lesion is seen in the lung on xray and ct scan then eventually a biopsy is required.
If this is the only spot then biopsy is done by ct guidance of a needle when the lesion is in the periphery or outer aspects of the lung. If the lesion is central then a bronchoscopy (fiber optic tube) into the long is used to get a needle biopsy and washing of the bronchus.
