In brief: Fracture

Pain, swelling, loss of motion or stiffness of the joint, deformity, difficulty bearing weight.
This can be difficult to differentiate from a severe sprain or contusion without an xray.

In brief: Bone out of skin!

But of course that would be obvious! usually severe swelling, inability to bear weight and point (focal) tenderness are suggestive signs.
If there is deformity then that makes the diagnosis easier. An x-ray is always needed to make the definitive diagnosis.
