In brief: Avoid the cause(s)

Cigarettes and pollution are the two biggest contributors.
Stop one and avoid the other as much as you can. Treatment is symptomatic; bronchodilators if you are short of breath, steroid inhalers for people who have frequent "exacerbations", antibiotics and oral steroids for acute exacerbations, and yearly influenza vaccination to reduce risk of serious illness. Oh, and did i mention "no smoking?".
