Prostate cancer that has spread often metastasizes to bones especially backbone.
As part of evaluation for high grade prostate cancer, your urologist or oncologist may recommend bone scan to look for evidence of spread before you develop pain. If you're already having bone pain, regardless of site, go see your doc immediately. Good luck!

In brief: Pain at the spot

Pain will ussually be at or close to the area where the metastasis is, especially if there is a fracture.
An exeception would be injury to a nerve that may cause pain distant to the site of injury (for example pain that shoots down the leg in lower spinal nerve damage). Note that patients with multiple sites of bone mets may not hurt the same or at all in each spot. Best to you.
