Cardiologist says heart is fine because of ekg/holter/echo/thyroid/adrenal good. Constant tachycardia/pacs/palps though. Ef 50%. Possible error?

It is extremely doubtful that you have a heart problem with all of that testing coming out normal.
To be honest, it is much more likely to be related to stress/anxiety in your life, or possibly due to less than optimal lifestyle behaviors (diet, caffeine, exercise frequency, etc).
