Although many elderly patients don't have bowel movements for 3-4 days normally, in this case the patient has a distended abdomen.
If she has had no b.M. In 4 days and her abdomen is distended, i would consider impaction and seek treatment as soon as possible.

In brief: Constipation/impacti
A simple digital rectal exam showing hard stool in the rectum makes the diagnosis.
Then the patient will need enemas or digital DES impaction ( removing the stool with the fingers). As a general rule a person must move its bowels at least every three days otherwise should be using a laxative at that time. Elderly pts may prevent constipation by taking milk of magnesia (magnesium hydroxide) two or three times a week
