What would be the cause of pvcs only with heart rates over 100?

In brief: Benign vs. malignant
Features of benign pvc's usually are: monomorphic (same shape), late cycle ( they come after t wave on ecg), non-consecutive (they come as single PVC followed by normal beat) and absent of structural heart disease.
Now if the pvc's are coming only above hr 100/min the first question do they the features above, the second question do they disappear with excercise and hr above 150/min?

Tachycardia needs to be evaluated by your pcp and cardiologist.
It could be hyperthyroidism, severe anemia, stimulants/cocaine use, anxiety, heart disturbances (atrail fibrillation/flutter, & other) , fever, heart disease/coronary artery ds, . Need ekg, labs and further testing depending on the results.
