In brief: No O
The most common causes would be: medication, alcohol/substance abuse, low testosterone, depression, performance anxiety, issues in the relationship, pornography, or possibly a sleep disorder. If he is diabetic, has peripheral vascular disease, or is on high blood pressure medications this could be responsible as well.
A thorough medical work-up may be in order.

In brief: No orgasm
The first thing you need to ask is what has changed since the last time he had one.
We're new medication given to him as a result of some disease such as depression high blood pressure diabetes etc. These things can interfere in orgasm. Has he been tested for hormonal conditions that can cause low testosterone low thyroid or even elevated prolactin levels of the pituitary. He needs evaluation.

In brief: Don't feel ugly!
It sounds like you are a wonderful and caring lover! don't feel ugly at all, a person caring for her lover as you are is beautiful! many things can cause impotence, none of which involves you.
The development of impotence in a man in his 40's can be caused by minor problems, but also by serious disease, for example cardiovascular disease or diabetes. He should really be evaluated by his doctor.
