Try a trial of vitamin B12 (cobalamin) first and have a physician to repeat lab to ensure adequate dose.
Also it may need additional therapies for cobalamin deficiency due to underlying diseases involving intestine or pancreas. Please check for Folic Acid deficiency as well and this may be related to nutritional deficiency. If you are a vegetarian, please take vitamin B12 daily.
