Pickwickian syndrome is "obesity hypo ventilation syndrome", and occurs when in severe obesity patients obstruct the airway and do not breath, and take very small shallow breaths while awake, this leads to elevated levels of the waste gas/ carbon dioxide and low oxygen levels. This combination causes heart problems ( Pulmonary hypertension, heart failure. Wide spread obesity has made more common

In brief: From obesity
"pickwickian syndrome" is an old term for "obesity hypoventilation syndrome.
" this is when the lungs can't process the carbon dioxide because of too much weight on the lungs due to obesity. Thus, the carbon dioxide levels rise in the blood. Most obese people don't get ohs, but some can.

In brief: No,
Obesity hypoventilation syndrome is still usually only seen in morbid obesity.
While obesity is an epidemic, morbid obesity with obesity hypoventilation is only present in those with bmis > 40, and usually only those with bmi >45.
