There are mainly 3 possibilities 1) root canal failure / re-infection of a previously root canalled tooth.
And 2) fracure or crack in the tooth &/or root , and 3) missed canal during root canal. Your endodontist , and using a microscope , can detect which one of there is the causative factor. Good luck.
