In brief: Conception

Is the fertilization of an egg by a sperm.
It doesn't matter whether a penis is involved or if it happens in or outside the body such as in vitro fertilization.

In brief: 2 interpretations

Most obstetricians count first day of conception as first day of last menstrual period before ovum becomes fertilised.
Others, including catholic doctrine is that conception occurs with union of sperm and ovum which is the begining of life. This occurs some little time after sex has occurred and sperm has had time to swim up through cervix & uterus and into fallopiann tube & conception takes place.

In brief: No

Conception is usually determined by a positive pregnancy test either blood or urine.
Pregnancy could be from coitus or fertility treatment. The catholic church defines conception as a fertilized egg. This allows the church to be consistent on its views regarding ivf, abortion, etc but is a definition that is different that what many others use.
