Take 50 mg flecanide, took 100:( stress test showed vent. Tach. Just below concern) for very frequent benign pvcs. Other meds not work. Risk too hi?

In brief: Need more eval
You need a thorough evaluation including coronary arteriography and cardiac mri. Flecainide is excellent for suppressing single, symptomatic pvcs but doesn't lower the risk (and may increase the risk) of sudden death in people with structural heart disease. The underlying cause should be sought and corrected. Ablation may well be necessary.

First pvcs do not have to be treated unless they are causing disconcerting symptoms or they are too numerous causing weakness of your heart pump.
If they have to be treated you should ask your cardiologist if ablation is an option. If this is not an option, have simpler medications such as beta blockers been tried. Flecainide is okay if no structural heart problems or coronary disease.

Underlying coronary artery dz should be sought and ruled out. Correcting electrolytes magnesium levels should be done. Thyroid levels checked All stimulants like caffiene cold remedies and energy drinks should be stopped.
