In brief: Observation

More than half the time, no cause for microscopic hematuria is identified.
Could be idiopathic" hematuria, which just means we can't find an explanation. If there is also protein in the urine, then eval by a nephrologist may be useful. If the amount of blood in the urine increases over time, or if there is ever visible blood (gross hematuria), then repeat evaluation is indicated.
