It depends on the reason for not having surgery.
If surgery is not done because of medical ineligibility then there are methods to control cancer symptoms- embolization, cryotherapy, oral chemotherapy, radiiation, etc. Without any treatment symptoms such as urinary bleeding, pain, weight loss, and general failure occurs over time. Discuss with your oncologist and urologist.

If the kidney cancer is early stage, and you can't have surgery because of other medical issues, then you can receive systemic treatment with drugs such as sutent (sunitinib) or Nexavar and/or local therapy such as radiation or ablation.
If you can not have surgery because the cancer is widespread (metastatic or stage 4), tehn same systemic therapy can be used. With no treatment, by its nature, cancer spreads.

There are different types of cancer out there.
Some of them grow slowly some of them really fast.Some of them will disemminate quickly some of them rarely do so. Best advice will be taken from an oncologyst , that will make the proper recomandations and what your expectations should be. Cancer untreated kills in the end.

Small tumors, <2cm diameter can be observed for a while, especially if advanced age or co-morbidities are a problem. Otherwise the cancer will enlarge locally and involve other organs which are touching.
It can also spread via blood stream to lungs, brain bones and opposite kidney. Also spread via lymphatics to lymph glands. Causes weakness + fatigue, blood in urine, abdominal pain+short of breath.
