Fat in skin or deeper most common, hernia in abdominal wall muscles possible, especially involving embryological lack of fusion in midline between rectus abdominal muscles, umbilical hernia (because lower) less likely, many other possibilities.
A hernia involving bowel (much less common in upper abdomen) can easily create partial bowel obstructions with associated transit problems, belching, etc.
