Medical history. Your anesthesiologist will need to do a full medical history. If you have lab tests, cardiac studies, doctors notes from outside facilities, it would help to bring those. A list of medications and allergies is essential. The more we know, the safer we can keep you. This applies for any anesthetic.
Not sure. To have the surgery performed, you need to show up and have the preop lab and xrays done. The surgeon will arrange this. You need to bring your insurance card, drivers license, and if the hospital requires some payment before surgery, you'll need to bring or pre- arrange that.
