Empyema is pus in the space surrounding the lung.
It usually requires drainage with a chest tube as well as antibiotic therapy. Fortunately, this is not a very common occurrence.

A classic example is a collection of pus within the lung pleural space, or even the appendix. So, given that the definition of empyema is very broad, physicians do certainly see this phenomenon.
