In brief: Complex
It is hard to eradicate tb.
Part of the answer involves poverty, housing, public transportation, hiv, travel and immigration. Another part has to do with the disease since it goes dormant and takes time to manifest when active. If we could magically stop all transmission today, we can still see cases for the next 75 years.

In brief: And world wide
It is a problem everywhere.
Everybody with a positive skin test may develop contagious tb. Probably about 10% of the above. And many are not checked or do not want to take the medication to protect them.
