Pt has contractures of group of muscles like when smiling ironically ( sardonic smile), and contractures of limbs. Life-threatening neurologic syndrome characterized by tonic muscle spasms and hyperreflexiatetanus toxin can be detected in serum, confirming a clinical diagnosis. Samples should be collected before immunoglobulin treatment.
