In brief: If necessary

Scrotal ultrasound is normal and indicated for following reasons:- scrotal pain, scrotal swelling, rule out testis torsion, suspected testis tumor, acute hydrocele, infertility, non-palpaable testicle in child, compare testicle size in presence of varicocele, follow-up for testicular microlithiasis.

In brief: No

No one should need it unless they have a problem like pain in testis, mass in testis, new enlargement in testis.

You could miss a testis tumor by witholding scrotal US in an adult or adolescent patient with a hydrocele. Test is non-invasive and potential benefits outweigh such considerations as cost!
