In brief: Usually not...

I will presume that you are talking about therapy for some type of cancer (prostate, breast, lymph nodes.
..). Radiation implantation itself may be uncomfortable, but generally is not painful. The radiation treatment (being released by the implant or externally) is also not painful. However there can be adverse effects that generally come months/years later that must be explained by the oncologist.
