In brief: No usually
People with itp usually bleed more frequently following gently trauma, like a bump.
In more severe forms, itp can be associated with gastric hemmorrages, which are painful, but this is rare.

Unlike ITP, TTP is usually presented with thrombotic events and not bleeding.

In brief: TTP
Ttp or thrombotic thrombocytopenic purpura can be manifested with seizure, coma, renal failure, cardiac hemmorhage/bleeding, sudden cardiac arrest, myocardial infarction, other thrombotic events such as DVT etc.
They will need to get treatment as soon as possible to survive. The pain can happen and usually is related thrombotic events -such as blood clots in deep vein, or heart attack.
