Infections are the most common reasons, especially chlamydia and gonorrhea, which are sexually transmitted diseases (stds).

Since a standing-alone orchitis is not common, I would need to know how you have carried the Dx of recurrent orchitis. Based on clinical recurrent scrotal pain or US finding alone or both? So, differentiating what might led to this Dx is needed by analyzing detailed Hx + physicals + tests as needed. So, seek pro-evaluation.

Orchitis by itself is usually single episode.
Eididymo-orchitis can certainly be recurrent. Infection/inflammation starts in the epididymis, which lies on top of testis, and inflammation spreads to testis.Often caused by sexually transmitted diseases otherwise caused by urinary tract abnormality allowing urine to reflux into ejaculatory ducts then via vas tubes to epididymis. Diagnosed by VCUG xray.
