My sister says she has a little puncture wound of scalp. Can she just shower off and clean it?

In brief: Poor Idea
Puncture wounds are dangerous because the bacteria from the skin and the object that caused the puncture are pushed into the deeper layers.
If the would is not cleaned thoroughly and managed properly then she could get a very serious, even life threatening, infection requiring hospitalization and IV antibiotics. She really needs to see a professional and get evaluated.

In brief: Yes
Soap and water. If it begins to swell of ooze, it should be checked by a doctor, but the vast majority of the time small wounds just heal if you keep them clean.
