In brief: Good PT

It would be helpful to work with a good pelvic floor physical therapist.
They are skilled in both internal and external work which may be necessary. Usually some muscles are too tight and other muscles are too weak. Others are in a constant state of contraction. A consultation with a physiatrist specializing in pelvic floor dysfunction may also be beneficial.
