In brief: Yes

Fatty liver can cause the liver to swell within it's fibrous capsule or coating.
The liver has no pain fibers, but the capsule does.

In brief: Doubt it

Anything that expands the capsule of the liver may theoretically cause pain but it is not common in fatty liver.

In brief: Rarely

Fatty liver (steatosis, steatohepatitis) is generally not painful.
There patients who note a dull achey feeling in the right upper quadrant. This can be quite bothersome at times. One theory is that an enlarged, fatty liver streches the glisson's capsule which goes around the liver.
