If this is really a pilonidal cyst, it needs to be incised and drained and washed out.
There are foreign objects inside the cyst (usually ingrown hairs) which are a nidus for infection. Creams will not treat this. You need to see a doctor who can properly diagnose the pilonidal cyst. Often they have complex pathways (tracks) which may require the services of a general surgeon.
