In brief: By sensitivities

Urine should have had a culture with bacterial sensitivities by micrbiology done, sensitivity testing should indicate choice of a few suitable antibiotics.
Nitrofurantoin should not be used, although effective at sterilizing urine does not achieve any tissue levels and thus isn't effective in treating pyelonephritis.

In brief: Needs urine culture

Selecting the most appropriate antibiotics requires culture of properly collected urine sample.
Pyelonephritis requires longer treatment as well.

In brief: UTI THERAPY

Your best bet is always to have the urine tested to see what kind of bug is causing the infection and find out the antibiotic that the bug is most sensitive to.
Of cause care must be taken to make sure you are not allergic to that particular antibiotic. Thankfully though most time you will have 2 or more antibiotics that have good activity against the culprit.
