In brief: Worth finding out
This could be caused by hormone problems, bleeding problems, growths inside the uterus, medicines you take or using an iud for birth control.
You are likely to be at risk for iron deficient anemia with that long a history. See your doctor, get blood tests and a pelvic exam.

Internal Medicine
In brief: Possibly fibroids
Your pattern of vaginal bleeding is not normal and should definitely be investigated by an ob/gyn.
You should have a pap smear and a pelvic ultrasound for starters. These may be signs of enlarged fibroids, or ovarian cysts, both of which can sometimes be seen by a pelvic ultrasound. Get evaluated soon because excessive bleeding can also lead to anemia and other symptoms.
