In brief: Efficient

Not enough info- but generally TMJ argthroscopy is more efficient.

In brief: Arthroscopic

Keeping the joint area closed during surgery is more efficient, whether it is for the tmj, knee, elbow, etc.
This allows for more rapid healing, and less post operative complications. As the TMJ is a more complicated joint (in terms of function) than other joints, i would expect that any efforts to minimize risks and complications would be the best course of action.

In brief: Depends

Depends on the skill of the surgeon.
If he can do it through scope, is better.
