A complete history and physical along with appropriate tests, including cbc, chem, thyroid tests including thyroid antibodies, cpk, rf, ana, parvo, ebv, and lyme. Depending on your history and risk factors additional studies may be ordered. Treatment starts with an accurate diagnosis.

To the above great list of Dr. Keler , I would add Sleep apnea to be ruled out as most patient with CFS have sleep symptoms and if they snore and have daytime sleepiness and or hypertension. Integrative medical care with standard medical care, diet, exercise, pranayama, relaxation, selective herbal supplements, good sleep hygiene all may be incorported to improve CFS. Discipline is a key.

Yes to the Sleep evaluation. I think asking about sleep in the history is another vital sign in rheumatology!

Causes like thryroid dysfunction , anemia, rheumatolgical cause, sleep problems, like sleep apnea, depression etc. Then get treated accordingly. To then add other modialites of optional treatments. Things like look into life style changes as appropriate with good healthy diet, regular 7-8 hours of sleep, relaxation, yoga, good bowel movement everday.

CFS is often considered a diagnosis of exclusion. This means that Dr. Viramgama's advice is excellent. The only proven cause for CFS is Herpes 6 infection, for which there is not yet any specific therapy. Depression may, in the opinion of many, be the major cause of chronic fatigue.
