In brief: Usually

Most extended release meds are more effective becuase the drug levels do not fluctuate much over 24 hours.
There is usually a big cost difference between the two and if insurance does not cover extended release short acting can still be used with good results. The intervention that will make the medication even more effective is a diet made of small meals, low in carbs and daily exercise.
