Commonly it involves disorders of the colon. It may be as benign as gas / pain or serious conditions such as diverticulitis or even malignancy. You should see a physician.

Could be gastrointestinal (gi), urologic or gynecologic.
Some of this depends on your age and what makes your symptoms better or worse. From a GI standpoint, does moving your bowels make your symptoms better or worse? Does eating affect symtoms? Is there a change in your bowel pattern? Weight loss? Blood in the stool? If any of these are present, it is likely the problem is GI in origin.
