In brief: Migraine or sinus

Pain behind the eye not associated with vision loss is usually due to migraine headache or sinusitis.
However, pain behind the eye with significant vision loss in the same eye needs to be evaluated as optic neuritis can present this way.

In brief: Sinus disease

Most complaints of pain behind the eye are from by nonocular causes.
I believe the most common is sinus disease followed by headache syndromes. It is almost never from high eye (intraocular) pressure as most patients think.

In brief: Lots of things

The most common cause for pain behind the eyes is sinus pressure.
It can also be caused by dry eye and a number of other things. The first thing i would do is to see your pcp to have your sinuses evaluated.
