In brief: Possibly CHF
Were your tof and bicospid av surgically corrected? If not there's no mystery why you should be getting progressive sob right.
If corrected, you are still at risk of developing heart failure. It could also be another pulmonary embolism. You need to discuss this with your physicians or go to er if you are really feeling bad.

Dr. Abe has given you good advice. This could be extremely serious and a physician visit is in order at once.

In brief: Check an echo
There could be any number of reasons for your symptoms, many which have little to do with your past history.
However, i'd recommend an evaluation and an echo to look for new problems with your valves (such as worsening leakiness after endocarditis) and to check pressures in your lungs to exclude pulmonary hypertension.

In brief: Too many
That is a very complicated situation.
The cases could be pulmonary and/or cardiac. Tof and bicuspid av alone could cause sob from anyone of at least 6 causes. You need to let you physician know.
