In brief: Knee fat pad hurt
Hoffa syndrome refers to what can happen if the kneecap recieves a direct blow.
Underneath the patella (kneecap) and the tendon connecting it to the lower leg is a fat pad. It can get pinched between the patella and the thigh bone. This can be very painful. Usually it gets better with rest, ice/heat and time. At times the fat pad can be so injured that surgery is needed.

In brief: Inflammation
Hoffa syndrom is inflammation/swelling in the fat pad below your patella (kneecap), deep to the patellar tendon.
It is usually treated conservatively with rest, stretching, and medication, but some tougher cases require surgery and removal of the inflamed fat.
