In brief: Seek Help

Beyond what you already know regarding diet and exercise, you may benefit from additional help from your internist, personal trainers, dietitians and/or weight control “specialists” in your area.
These professionals will be of more help to you than any noninvasive or invasive procedure. Best wishes.

In brief: Lose weight

There are no substitutes for diet and exercise particularly in a 45 year old overweight male smoker who is at particularly high risk for a heart attack.
Au localized approach such as cool sculpting is really not wise at this point unless you are btmcncerned abut your overall health. By the way, your history says c-section, how does a male have one?

In brief: Difficult....

The non-surgical methods tend to be quite limited in what they can do.
Have you considered coolsculpting?
